1. Topical Treatments:

  • Vitamin D Analogues: These topical agents regulate skin cell growth and reduce inflammation. While they’re generally well-tolerated, they may cause irritation or itching in some individuals.
  • Retinoids: Derived from vitamin A, retinoids slow down the growth of skin cells and alleviate inflammation. However, they can cause skin dryness and irritation, particularly during the initial stages of treatment.
  • Calcineurin Inhibitors: These agents suppress the immune response in the skin, effectively reducing inflammation and plaque formation. They’re particularly useful for sensitive areas like the face and genitals but may cause a burning sensation upon application.
  • Coal Tar: Known for its anti-inflammatory and anti-itch properties, coal tar preparations can effectively reduce scaling and inflammation. However, their strong odor and potential for skin irritation may limit their use.

2. Oral or Injectable Medications:

  • Methotrexate: This oral medication works by inhibiting the rapid growth of skin cells and suppressing the immune system. While effective, it requires regular monitoring due to potential liver toxicity and other side effects.
  • Cyclosporine: Another immunosuppressive agent, cyclosporine, can provide rapid relief from psoriasis symptoms. However, its long-term use is limited by the risk of kidney damage and hypertension.
  • Apremilast: A newer oral medication, apremilast, targets specific inflammatory pathways involved in psoriasis. It’s generally well-tolerated but may cause gastrointestinal side effects in some individuals.
  • Biologics: These injectable medications target specific components of the immune system responsible for driving psoriasis inflammation. While highly effective, biologics carry a risk of serious infections and require regular monitoring.

3. Light Therapy:

  • UVB Phototherapy: Exposure to UVB light slows down the rapid growth of skin cells and reduces inflammation. UVB phototherapy can be administered in a dermatologist’s office or at home with specialized equipment.
  • PUVA (Psoralen plus UVA): This therapy combines the use of psoralen, a photosensitizing agent, with UVA light exposure.

While effective, PUVA therapy requires careful monitoring to prevent side effects such as sunburn and an increased risk of skin cancer.

Several factors should influence the selection of a psoriasis treatment, including:
